What Is FibroScan?

FibroScan (also known as transient elastography or liver elastography) is a specialised, non-invasive medical test that measures the stiffness of your liver. Liver stiffness is a direct, reliable indicator of the amount of fibrosis (scarring) and fat accumulation present in the liver — making FibroScan one of the most valuable tools available for assessing liver health without the need for a biopsy.

The test works by sending a mild vibration pulse through the liver using an ultrasound probe placed on the skin over the rib cage. The speed at which this pulse travels through the liver tissue is measured — stiffer liver tissue (indicating fibrosis) transmits the pulse faster. This measurement, expressed in kilopascals (kPa), directly corresponds to the degree of liver scarring.

Additionally, the FibroScan device measures the Controlled Attenuation Parameter (CAP) score — which quantifies the amount of fat in the liver, providing a simultaneous assessment of fatty liver disease.

Understanding Your FibroScan Results

FibroScan results are expressed using the METAVIR fibrosis scoring system, which classifies liver fibrosis from F0 (no fibrosis) to F4 (cirrhosis). Here is what the numbers mean:

In the majority of cases, FibroScan provides all the information needed to stage liver disease and guide treatment. Liver biopsy is reserved for situations where the diagnosis is genuinely uncertain, or where knowing the exact degree of liver inflammation (not just fibrosis) will change your management plan.

What to Expect During Your FibroScan

1. Preparation

Fast for at least 2 hours before the test. Wear loose, comfortable clothing that allows easy access to the right side of your abdomen.

2. Positioning

You will lie on your back on the examination table with your right arm raised above your head to expose the right side of your rib cage.

3. The Test

A gel is applied to the skin over your liver. The FibroScan probe is placed on the skin and a series of 10 measurements are taken. You will feel a gentle vibration — the procedure is completely painless.

4. Duration

The test takes just 5–10 minutes. You can return to all normal activities immediately afterwards.

5. Results

No — FibroScan is completely painless. You will feel a gentle vibration on the skin over your liver, similar to a light buzz. There are no needles, no incisions, and no sedation required. Most patients find the procedure quick and completely comfortable.

FibroScan has excellent accuracy for staging significant fibrosis (F2+) and cirrhosis (F4), with diagnostic accuracy of 80–90% compared to liver biopsy. Results may be less reliable in patients with high BMI, severe inflammation, or early fibrosis (F0–F1). Your specialist will interpret results in the context of your overall clinical picture.

For patients with stable chronic liver disease, FibroScan is typically repeated every 1–2 years. Patients undergoing active treatment (e.g., antiviral therapy for hepatitis or lifestyle modification for fatty liver) may benefit from more frequent testing — every 6–12 months — to monitor treatment response.

You should fast for at least 2 hours before the test — eating causes increased blood flow to the liver, which can falsely elevate stiffness readings. A 3-hour fast is ideal. You can drink water normally.
